Communify - HART 4000 - Fortitude Valley

We are an assessment and referral team working closely with anyone who may be homeless or at risk of becoming homeless in Brisbane.

Footprints Community - Assistance with Care and Housing

The Assistance with Care and Housing (ACH) program is designed to support people who are homeless or at risk of homelessness, to access appropriate and sustainable housing as well as community care and other support services, specifically targeted at avoiding homelessness or reducing the impact of homelessness. ACH also provides support to people experiencing symptoms of hoarding disorder or who may live in domestic squalor.

Footprints ACH program acknowledges the value of lived experience. This program offers peer support work alongside case work and case management, to assist with housing and/or decluttering needs.

ACH is a free service, funded by the Australian Government as a part of the Commonwealth Home Support Program.

Footprints Community - Queensland Community Support Scheme

The Queensland Community Support Scheme (QCSS) provides support to individuals to maintain or regain their independence, continue living safely in their homes, and actively participate in their communities.

You may be eligible for the QCSS if you are under 65 years old (or under 50 years old for Aboriginal or Torres Strait Islander people) with:

• a disability (and are not eligible for the National Disability Insurance Scheme)
• chronic illness, mental health or other condition, or
• circumstances that impact your ability to live independently in the community.

Footprints Community - Stand Up, Step Out

Footprints SUSO outreach bus and its dedicated team is here to support individuals that are sleeping rough and/or at risk of homelessness. We can provide those individuals with a comfortable and safe environment to shower, access laundry facilities, and our dedicated team is available to help individuals get access to the right supports and services.

SUSO works with organisations and neighbourhood centres to complement the existing services that are already present in the area.

Our service currently operates weekly in Wynnum, Capalaba, Lawnton, Zillmere and Clontarf, over a 5 day working week.

Metro North - Mental Health RBWH

We provide a range of assessment, treatment and support services for people and their carers’ who are experiencing mental health or drug and alcohol issues.

Open Doors Youth Service Inc.

Open Doors Youth Service provides support to young people with diverse genders, sex and sexualities.
They support young people who identify as Lesbian, Gay, Bisexual, Transgender, Intersex, Queer, Asexual, Pansexual, Sistergirl or Brotherboy (LGBTIQAP+ Sistergirl & Brotherboy) aged 12-24 across South East Queensland.
They are based in Fortitude Valley, Brisbane but provide outreach in varying capacities.
They are Queensland’s leading LGBTIQAP+ youth homelessness service, working with young people who are at risk of/are disengaging with community and are at risk of/are experiencing homelessness. They aim to support and stabilise living situations and provide early intervention to support finding a safe living arrangement. This may include finding accommodation short/long term, supporting in providing mediation with parents, schools and other family members as well as advocacy and providing referrals and links to safe and supportive services.

Metro North Health enables people to deliver excellent patient-centred care and high quality health services.

Somerset Villas Community Care Unit - RBWH

Somerset Villas is an Extended Treatment and Rehabilitation Facility which provides recovery based services to individuals who experience mental illness and require 24-hour clinical support within a community residential setting. The focus of this service is to enable residents to develop the skills required for independent living, with an emphasis on both skill development and community integration.

Valley Hearts Inc

A Christian faith based group who support and walk with the marginalised in the Fortitude Valley area, assisting with housing & materials needs, faith activities and community meals. Partnering with other groups, eg. Red Cross Jeays Street Community Centre, Wesley Mission Brisbane & SPEAR Ministries to further strengthen and support this community.

Wesley Mission Brisbane - Emergency Relief

The Emergency Relief service provides support to people who are homeless and/or at risk of becoming homeless, to individuals and families who are marginalised and economically and socially disadvantaged. ER assists people in times of crisis with food parcels and vouchers, with urgent bill and rent payments (when funding allows), with payments for scripts for essential medications. The ER Service also assists families with much needed baby clothes and items such as prams, cots, nappies, formula. Case mangement and counselling (information and referral) is also offered to clients accessing the service.

Eating Disorders QLD

Eating Disorders Queensland supports a vibrant community of individuals recovering from an eating disorder, including people with lived experiences, carers and family members.

Emmanuel City Mission

The mission of Emmanuel City Mission (ECM) is to connect the homeless and vulnerable with people who care; increasing trust and decreasing loneliness.

ECM is located in a large warehouse-like space in South Brisbane where homeless and vulnerable people can eat a healthy meal, rest, wash clothes, have a shower, be out of the weather, be safe and very importantly connect with people that genuinely care about them. Safe relationships can be formed even of only for the day. ECM is open 7 days a week and people can trust that we will be here for them tomorrow.

ECM also offers space for other outreach service providers free of charge to foster a diversity of connections and also enable a person to seek a pathway beyond their current circumstances.

Similarly, ECM seeks to build partnerships with other local community organisations (including schools, parishes and local businesses) to improve services to the homeless while also building connections to the vulnerable in the community.

Micah Projects - Common Ground

Integrated healthcare is a key component of Micah Projects supportive housing services provided at Brisbane Common Ground. Nurses employed by Micah Projects, and funded by St Vincent’s Private Hospital Brisbane and Mater Health, provide seven-day-a-week nursing care as part of the Supportive Housing Hope Street team at BCG.

Our activities have contributed to preventing and reducing homelessness; increasing family support to vulnerable families; providing support services and support for the National Redress Scheme for people who have experienced abuse in institutional settings; continue to work towards reducing social isolation for individuals and families as well as increasing economic participation through training and employment.

Murri Watch Aboriginal and Torres Strait Islander Corporation

Murri Watch is a not-for-profit incorporation established in 1991 just prior to the handing down of the Recommendations arising from the Royal Commission into Aboriginal Deaths in Custody. Murri Watch is determined to stop Aboriginal and Torres Strait Islander deaths and self-harm in custody, and to deliver high quality services and support options for Aboriginal and Torres Strait Islander people who are homeless, or at risk of homelessness. Murri Watch delivers a range of programs across the state. These programs include Diversion from Custody, Cell Visitor Services, Community Patrol Services and Homelessness. Our Homelessness Service, Bowman Johnson Hostel is a 20-bed hostel which provides short term accommodation combined with case management services for homeless person or persons at risk of being homeless.
